What does the NHS surcharge cover?

Prescriptions and dentistry
NHS prescriptions cost £9.90 per item (or free for those on qualifying benefits). NHS dental treatment follows standard NHS charges — not full private rates.
Maternity care
Maternity services are fully covered by the NHS surcharge. You will not be charged for ante-natal care, labour and delivery, or post-natal support.
What it does not cover
Assisted conception / IVF, cosmetic procedures, and some long-term social care are not covered. Some specialist services may require prior approval.

The standard NHS Immigration Health Surcharge is £1,035 per person per year in 2026. Students and Youth Mobility Scheme applicants pay a reduced rate of £776 per person per year.
How is the NHS surcharge calculated?▾
The surcharge is charged in 6-month periods. Each 6-month period (or part thereof) costs half the annual rate. For example, a standard 2-year 6-month visa costs 5 × £517.50 = £2,587.50 per person.
Do dependants pay the NHS surcharge?▾
Yes. Each dependant — including your partner and children — must pay the NHS surcharge at the same rate as the main applicant. All amounts are paid upfront together when you apply.
When do I pay the NHS surcharge?▾
The NHS surcharge is paid upfront at the point of application — before your visa is approved. You pay for the full duration of the visa you are applying for.
Can I get a refund on the NHS surcharge?▾
You may be entitled to a refund if your visa application is refused, withdrawn, or if you are granted a shorter visa than you applied for. Once you become a British citizen or get ILR, the surcharge you paid for any remaining period may also be refunded.
